* Parents: Mary Storch and Robert B Ballman.
* Married 26 Sep 1900, in Middletown, to Sarah Bodine (daughter of Mary Wiggins and John H Bodine). Survived by Sarah.
* They had four children: J Allan, Elizabeth, Ruth, and Marion. Survived by his children.
* Survivers also included a sister, Mrs Anna Legfard of Camden, NJ, and a brother, George Ballman.

* 23 Jun 1898 - 20 Feb 1899 served in the Spanish-American War into Company I, New York Volunteers and served as a Private in both the United States and Hawaii.
* Re-enlisted almost immediately into the 24th Separate Company, New York National Guard as a private. Discharged as a Sergeant, 27 Mar 1906.
* Enlisted the same day into the 1st New York Infantry. He transferred to the reserves, 26 Jan 1915, after rising to the rank of Captain and Commander of Company I.
* 27 Jun 1916 returned to active duty in the 1st New York Infantry for the Mexican Expedition.
* 10 Aug 1916 entered into Federal service as a Captain to recruit from New York City and Orange County. Returned to reserves on 15 Nov 1916.
* 15 Jul 1917 went back into active service in Company F, 1st Infantry, New York National Guard for the World War. He served at various US posts and in France. Discharged 19 Feb 1919 with a physical disability. There are many more details of his military career in the book listed below, but the cause or extent of his physical injury isn't given.
* 1920 appointed Armorer at the New York State Armory in Middletown and stayed active in the New York National Guard until his death.

He suffered "a stroke of paralysis" in Aug 1929 from which he never recovered. In his biography, the cause of the stroke was attributed to his World War experiences. He died at his home at 18 Bonnell Street.
His funeral was held at the Christ Universalist Church, led by Rev E. W. Whippen. He had a military funeral with a firing squad, military escort, and Masonic rituals.

Captain Jacob S. Ballman of Middletown, veteran of the Spanish American War and World War, died at his home in that city and was buried Monday, according to information received by local men who served with him in the First Pioneer Infantry during the latter conflict. Ballman, who was widely known in veteran and National Guard circles, was active in work of the American Legion and United Spanish War Veterans, and had many friends in this locality. During the World War he served with an anti-aircraft machine gun company In France, and was commander of Company F of the First Pioneer Infantry a later period.
